Getting Ready for Your Visit

The offices of Dr. J. Moritz, Dr. John Urbancic, and Dr. Melanie Carlson. All physicians are Board Certified and this medical office is Nationally Recognized as a Patient Centered Medical Home.

Receive your flu vaccine in the months of October - December so it will best protect you during the flu season. Call the office to schedule your fluvaccine.

We appreciate the opportunity to provideyou with medical services.  We serve patients of all ages, from infants to mature adults.  The information that follows is designed to answermany of the questions that most patients have.

Press "1" for Appointments

When calling for an appointment, the receptionist willrequest your name, telephone number, the nature of your visit and insurance information.  This is to help insure the efficient use of the doctor’s time and maintain the highest quality of healthcare for our patients. Please be prompt for yourappointment.  If you are ill and call for a same day appointment we will do our best to accommodate you.

We respect your time. In the event more problems are discovered during the examination than those for which you were originally scheduled, it will be necessary to schedule a new appointment to address those problems. In this way, your new medical issue is given thoughtful consideration and will not disrupt other patients' appointments. 

Press "1"  for Questions

Sometimes you may call with questions while the doctor  istreating patients.  He or shewill not be able to take your phone call, so please leave your message with ourstaff. They will call you back with the doctor’s reply by the close of thenext business day.  If calling about lab or otheroutpatient studies, please allow 7-10 days for the medical staff to contact you through SecuReach (our Medical Communications System) about the results.      

After Hours

To contact a doctor afteroffice hours, please call our answering service (440) 646 - 8728.   All urgent messages will be forwarded to the physician.

Press "4" for Referrals

Insurance referral requests should be made on the referral voice mailbox, including the following:
   -Your name, patient's name & patient's date of birth
   -Your relationship to the patient
   -Name of insurance & reason for referral
   -Daytime phone number and location of doctor or facility requiring referral
   -Date of service is needed

Press "6" for Business & Billing Questions

We might need to call you for information to insure your claim is processed quickly and correctly. You may leave pertinent billing information in our secure voice mail system. Please leave the following information:
   -Your name, phone number and relationship to the patient
   -Patient's name, date of birth, name of insurance, insurance ID, and any billing questions
    you may have.


Please call the pharmacy when requesting refills. The pharmacy will contact your doctor. If possible, we find it much safer if you have all of your prescriptions filled at the same pharmacy so that one place has a record of your medication profile to detect drug interactions and duplicate therapy.


If you have insurance coverage, please provide our office with your current I.D. In many cases, we will bill your insurance company directly. However, the patient is ultimately responsible for medical charges. If you are not prepared to pay the co-payment amount, we may ask you to reschedule your appointment or assess a billing charge of $10.00. Co-pays, per insurance company guidelines, are payable at the time of service. We accept cash, check, Visa, Mastercard.


SecuReach provides a secure voice mailbox for patients to receive test results and communications from your doctor in a timely manner. Each patient will receive a wallet-sized card. The SecuReach operator will call you as soon as there is a message. You should then call the 800 number on your personal card to retrieve your message. Please keep this card with your insurance card.

Self-Pay Patients

New self-pay patients are required to pay a $100.00 deposit before seeing the doctor. You may be asked to re-schedule the appointment if it is not paid. When you make a full payment on the day of your office visit, you will receive a 20% courtesy discount. You will continue to receive this discount if you maintain a zero balance.

Collection Policy

Unpaid balances over 60 days old will be sent to a collection agency unless a payment plan has been signed. This will result in a dismissal from the practice and a $15.00 collection fee.

Emergency Care

Go immediately to the emergency room of the nearest hospital in the event of an emergency. Provide them with the name of your primary care doctor. If you are hospitalized, call Geauga Family Physicians when you are discharged to schedule a hospital follow up appointment.


We ask that you provide us 24 hours notice if you choose to cancel an appointment. This allows us to accommodate another patient's needs. There will be a $25.00 charge for a missed office appointment and a $50.00 charge for a missed physical appointment. 

Method of Payment for Services

We accept cash, check, Master Card, and Visa.
Self Pay Patients:  Self pay patients are required to pay a $65 deposit before seeing the doctor.  The balance of the visit charge is paid afterthe appointment.  You may be asked tore-schedule the appointment if the deposit is not paid.  When you make a full payment on the day ofyour office visit and your previous balance is zero, you will receive a $10.00courtesy discount.
